Your card
The details MetLife asks for
MetLife dental plans are almost always through an employer, and many don't come with a physical card. If you don't have a card, these details work just as well:
- The subscriber's name and date of birth (the employee, if you're a dependent)
- Member ID or the subscriber's ID
- The employer or group name
- The plan type, usually listed in your benefits paperwork
Plan types
PPO plans and managed care plans
Most MetLife dental plans are PPO plans, which let you choose your dentist and pay less in network. MetLife also offers managed care dental plans in some areas, which usually require you to use an assigned dentist and work from a copay schedule.
If your benefits paperwork mentions a DHMO or managed care plan, call us before booking and we'll check how it works with our office.
Coverage
Timing your treatment around the yearly maximum
Every dental PPO plan has a yearly maximum, the most it will pay in a benefit year. Unused benefits generally don't roll over. If you have treatment to do, knowing how much is left, and when your benefit year resets, can save real money.
We check what you've used so far and can suggest how to stage treatment, for example finishing urgent work this year and planning the rest after your benefits reset. Every option comes with its cost in writing.
Coverage details are typical examples, not a guarantee. Your plan documents and our verification are what count.
How we verify
What happens after you send your details
- We contact MetLife (or check its provider portal) to confirm your plan is active and in network with us.
- We note your coverage levels, deductible, what's left of your yearly maximum, and any waiting periods.
- The front desk calls you back with what your first visit should cost.
- If you need treatment, you get a written estimate before anything starts.
